Birth certificate innacurate

here's an interesting one , my grandmothers brother had the wrong mothers name on his birth certificate , he has his grandmothers name ... i can only imagine that when my great-grandfather was asked for the mothers name he had
brain-fade and said his own mothers name :!: :!: has anyone else experienced anything like this ?

Thomas Ormerod (tommy) born in accrington lancs 1928 to father James Ormerod and mother who should've been Mary Hannah Ormerod (nee Devereaux) but on the birth certificate the mothers name is Elizabeth Ann Ormerod (nee crook) which his Jame's mother i.e Thomas 's grandmother

Thomas has siblings Annie , Jim , Doris , Doreen ,Joyce and Maureen , however they spelling of mothers maiden name "Devereaux" varies

is elizabeth husband also called james? is tommy the oldest ? is it poss that elizabeth is his mother and brought up by an older sister as his mother, as elizabeth couldnt manage ?

No James's father was also called Thomas, he was the 4th child (i think) of the family all the others have the correct mothers name .Elizabeth would have been around 50yrs old at the time
