Son of Mr J. and Mrs R. Akins, of Tipton, Staffs.
Born: Tipton, Enlisted: Dudley, Resident: Tipton.
First landed France & Flanders, post 31st December 1915.
Medal entitlement: British War Medal, Victory Medal.
Soldier's Papers at National Archives did not survive.

A Dependant's Pension Card exists in the name of John's brother - Joseph Akins of 7 House 6 Court, Wood Street, Tipton. There is no evidence that such a pension was ever paid.

Do I add John as his mother's maiden name Davies or as Akins as the death records have recorded ?

It needs to be birth name. So the children before marriage Davies.
